The Chance Foundation – We Are Together, based in Warsaw, exercises the utmost care in protecting your personal data, as outlined below.
At the outset, we would like to explain what GDPR is.
On 25 May 2018, new personal data protection regulations (GDPR) came into force. We would therefore like to explain what this means for you in practice. The purpose of this information is to present the full scope of your rights under personal data protection law, as well as the obligations we must fulfil towards you, including those arising from Article 13 of the GDPR (the so-called information obligation).
GDPR stands for Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, repealing Directive 95/46/EC. Whenever we use the term “GDPR”, we refer to this legal act. We may also use the term “General Data Protection Regulation” interchangeably.

The Chance Foundation – We Are Together processes personal data in accordance with applicable laws, in particular Regulation (EU) 2016/679 (GDPR).
Personal data are processed in accordance with the following GDPR principles:
a) lawfulness, fairness and transparency;
b) purpose limitation;
c) data minimisation;
d) accuracy;
e) storage limitation;
f) integrity and confidentiality;
g) accountability.
§ 2. Data Controller
The controller of your personal data is the Chance Foundation – We Are Together (details as above).
The Data Protection Officer (DPO) is the person you may contact with any questions or concerns regarding the processing of your data. This role is performed by:

§ 3. Purposes of Data Processing
The Foundation processes personal data in particular for the following purposes:
- participation in modern rehabilitation projects carried out by the Foundation;
- management of participation in events organised by us;
- sending information about our activities;
- conducting recruitment processes;
- issuing invoices, bills, and maintaining financial records, including accounting and payment processing;
- performance of civil law contracts;
- promoting equal life opportunities for people with disabilities through modern rehabilitation and social adaptation.
§ 4. Legal Basis for Processing
Personal data are processed on the basis of:
- Article 9(2)(a) and Article 6(1)(a) GDPR – consent;
- Article 6(1)(c) and Article 9(2)(j) GDPR;
- Article 6(1)(c) in conjunction with Article 7(1) and (3) GDPR, and Article 9(2)(j) GDPR (e.g. for archiving purposes);
- Article 6(1)(b) GDPR – for the performance of contracts.
§ 5. Data Sharing and Entrustment
The Foundation does not sell or disclose personal data except where required by law or described in this Policy.
Data may be shared with service providers acting on our behalf under Article 28 GDPR, including:
- IT and technology service providers;
- logistics and organisational service providers (e.g. transport, logistics support, security services).
Your personal data will not be transferred outside the European Economic Area.
§ 6. Data Retention Period
We retain your data only as long as necessary:
- where based on consent – until it is withdrawn;
- where based on legal obligations – for the period required by law (e.g. 5 years for tax/accounting);
- after fulfilment of the original purpose – for archiving or legal claims;
- for claims – until the expiry of limitation periods.

- rectify inaccurate data;
- erase data (“right to be forgotten”);
- restrict processing;

Handling requests and complaints related to accessibility
Everyone has the right to request the digital accessibility of a website, mobile application, or any of their elements. If the public entity refuses to fulfill the request to ensure accessibility or an alternative means of access to information, the requesting person can file a complaint regarding the digital accessibility of the website, mobile application, or their element. After exhausting the above procedure, you can also submit a request to the Ombudsman.

- You can use a parking space. We have designated 2 parking spaces for disabled people.
- The parking spaces are painted blue and properly marked.
- The distance from the parking space to the building entrance is about 20 meters.
- The parking is free of charge.

- There are stairs leading to the entrance.
- There is a handrail by the stairs.
- A permanent ramp with a 1% slope leads to the entrance.
- Opening the door does not require much force.
Horizontal pathways
- Rooms are located on 2 floors.
Vertical pathways
- You can reach other floors using stairs.
- You can use the elevator.
Elevator
- You have to open the doors manually.
- The elevator is located inside the building.
Rooms
- All rooms are accessible for wheelchair users.

- The building has a toilet accessible for disabled people.

Additional Information
We have facilities to improve communication – an induction loop, the reception area is equipped with an intercom, a device for reading printed text, an electronic magnifier, and a speech program. It is also possible to use the services of a sign language interpreter.
